What Are the Main Benefits of Reishi Mushroom?
Reishi's evidence splits into three unequal tiers: a real signal for immune-cell activity, an early signal for sleep, and no shown effect for blood sugar, blood pressure, or cholesterol. It contains compounds, mainly beta-glucans and triterpenes, studied for immune activity, sleep, metabolic markers, and adjunct cancer support, but the evidence for each isn't equal, and it isn't all the same kind of evidence.

Does Reishi Support the Immune System?

The extract used was a standardized, dosed beta-glucan preparation, not a generic mushroom powder, and not the same as whatever extraction and dose shows up in a random supplement blend on a shelf [1]. "Reishi increases immune-cell markers in healthy adults over 12 weeks at a specific studied dose" is accurate. "Reishi boosts your immune system" is not the same sentence, even though it gets written that way constantly.
Does Reishi Help With Sleep?
Reishi has a long traditional association with calm and sleep, but modern human evidence is still preliminary [8]. An animal study found that an acidic extract shortened sleep latency and extended sleep time in mice, an effect linked to increased serotonin signaling and shifts in gut bacteria [2]. A mouse study is not a human outcome, and there's no established way to translate a mouse dose into a human one.

On the human side: a study of 218 adults with chronic insomnia tested reishi extract (980 mg nightly, standardized to 6% triterpenes) against 5mg of melatonin over 8 weeks, reporting, in conference results not yet peer-reviewed, that reishi reduced insomnia severity scores more than melatonin did, with no serious adverse events [3]. That's a promising result, not a settled one, until it clears peer review and replication.
Put together, that's strong traditional use plus one early, unreplicated human signal. Does Reishi Lower Blood Sugar or Cholesterol?
Current human evidence does not support reishi as a meaningful treatment for high blood sugar, blood pressure, or cholesterol.
An 84-person randomized, placebo-controlled trial gave adults with type 2 diabetes and metabolic syndrome 3 grams of Ganoderma lucidum daily for 16 weeks, finding no significant effect on blood glucose measures (HbA1c, fasting plasma glucose) or secondary cardiovascular risk factors [4].
A more recent GRADE-assessed meta-analysis, pooling 17 randomized controlled trials and nearly 1,000 participants, found significant reductions in BMI, creatinine, and heart rate, and an increase in the antioxidant enzyme glutathione peroxidase, but no significant effect on body fat, waist circumference, blood pressure, fasting glucose, lipid profile, inflammatory markers, or liver enzymes [5].
Two separately conducted bodies of research land in the same place. The modest metabolic effects that did show up (BMI, creatinine, heart rate) shouldn't be read as a green light for the cardiovascular claims made far more loudly [5].
Is Reishi Safe? Liver, Interactions, and What's Still Unknown
Reishi appears generally well tolerated in short-term human studies, with a few honest caveats worth knowing rather than glossing over.
The federal LiverTox database, maintained by the NIH, describes reishi as generally well tolerated, without an association with routine liver enzyme elevations, while also noting rare, single case reports of acute liver injury where other causes weren't always fully ruled out [10]. That's rare case reports with unclear causality, not a pattern of proven harm.
On interactions, Memorial Sloan Kettering notes, in general terms, that herbal supplements can interact with medications [9]. That's a documented general caution, not a documented specific interaction: no dedicated human study has tested how reishi interacts with blood thinners, immunosuppressants, or diabetes medications. There's also a longer-view gap: the human trials behind reishi's benefits ran 8 to 16 weeks. Nobody has published long-term daily-use safety data, an absence of data, not evidence of risk.
What Dose of Reishi Has Been Studied?
There is no single proven reishi dose. Studied amounts vary widely by trial, and a milligram number on a label tells you less than it looks like it does.
The FDA's labeling rules for dietary supplements require just five things on the label: identity, net quantity, the nutrition/Supplement Facts panel, ingredient list, and manufacturer information [11]. Beta-glucan content, extraction method, and third-party testing aren't on that list. All of that is voluntary, so a label can say "reishi extract" and legally tell you almost nothing about what's inside.
Even when beta-glucan content is measured, the number depends on which lab method was used; a study of wild mushroom species found two validated testing methods producing meaningfully different results for the same samples [12]. Two products claiming the same beta-glucan percentage may not be comparable at all.

Reishi vs Ashwagandha: Which Is Better?
There is no direct human trial comparing reishi and ashwagandha, so any claim that one is better than the other is not evidence-based.
